If you are a professional working with the child / family, complete the online Professional Referral Form.

Upon completion

When you submit a completed form an acknowledgement page is displayed. The acknowledgement page displays the form’s unique reference number, you may wish to quote this if you contact the Children’s Duty Team about the referral.

Saving a copy

If you wish to print a copy of the form you must do it by clicking the button on the acknowledgement page. Once you leave the acknowledgement page you will not be able to return to it to print a copy of your form.



Allegations against staff or volunteers

Children can be subjected to abuse by those who work with them in any setting. All allegations of abuse or maltreatment of children by a professional, staff member, foster carer, or volunteer must therefore be taken seriously and treated in accordance with consistent procedures.
LSCBs have responsibility for ensuring there are effective inter-agency procedures in place for dealing with allegations against people who work with children, and monitoring and evaluating the effectiveness of those procedures (Working Together March 2010, 6.32) – see Chapter 3 of Working Together (March 2010)
